How to send us your documents
Please include your full name, date of birth and full address including postcode when sending us your documents. This will allow us to locate and update your record.
If your documents are required for a specific appointment please let us know the date and time of that appointment when sending your documents.
Send your email to firstname.lastname@example.org.
Documents should be sent as a .PDF file and pictures as a .JPEG file.

After you’ve sent us your documents
Once we receive your documents, we’ll usually upload them to your record within 3-5 working days.
We do not keep physical documents after they are uploaded but they will be available on your medical record.
